In consideration of my use of the exercise equipment and facilities provided by R3BEL FITNESS, LLC, I, {name}, expressly agree and contract, on behalf of myself, my heirs, executors, administrators, successors and assigns, that R3BEL FITNESS, LLC and its insurers, employees, officers, directors, and associates, shall not be liable for any damages arising from personal injuries (including death) sustained by me, or my unauthorized guest in, on, or about the premises, or as a result of the use of the equipment or facilities, regardless of whether such injuries result, in whole or in part, from the negligence of R3BEL FITNESS, LLC.
By the execution of this agreement, I accept and assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type, which may occur to me or my unauthorized guest, and I hereby fully and forever release and discharge R3BEL FITNESS, LLC, its insurers, employees, officers, directors, and associates, from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out the use of said equipment and facilities.
I expressly agree to indemnify and hold R3BEL FITNESS, LLC harmless against any and all claims, demands, damages, rights of action, or causes of action, of any person or entity, that may arise from injuries or damages sustained by me or my unauthorized guest.
I agree to be solely liable for safety and well being of any unauthorized guest I might bring into the facility and myself. I understand that R3BEL FITNESS, LLC does not provide supervision, instruction, or assistance for the use of the facilities and equipment. In addition, I agree to be liable for any damages caused to the facility and/or equipment by any unauthorized guest I might bring into the facility and myself.
I agree to comply with all rules imposed by R3BEL FITNESS, LLC regarding the use of the facilities and equipment. I agree to conduct myself in a controlled and reasonable manner at all times, and to refrain from using any equipment in a manner inconsistent with its intended design and purpose.
I understand and acknowledge that the use of exercise equipment involves risk of serious injury, including permanent disability and death.
I understand and agree that R3BEL FITNESS, LLC is not responsible for property that is lost, stolen, or damaged while in, on, or about the premises.
I understand and agree that my use of the facilities and equipment is only to be undertaken on my own personal time, and that my use of the facilities and equipment is not within the course or scope of my employment.
R3BEL FITNESS, LLC or any party designated by R3BEL FITNESS, LLC may photograph or film me while I attend the premises of R3BEL FITNESS, LLC and use of any and all such photos, video footage and/or video streaming for promotion, sales, publicity and advertising purposes for all media, including, but not limited to, the internet.
I HAVE READ THE FOREGOING WAIVER AND RELEASE OF LIABILITY AND VOLUNTARILY EXECUTED THIS DOCUMENT WITH FULL KNOWLEDGE OF ITS CONTENT.

Thank you for joining #TeamR3BEL, we ask that you abide by a few rules to promote the success of our lifting community. These rules are outlined below. We kept the rules brief as we realize that you probably won’t read everything if we go into too much detail. Please do us the favor of initialing next to each guiding statement indicating that you understand what it means and agree to incorporate it into your brain stuff. If you have a question about something not listed, ask another member. Together, you will probably come up with a reasonable answer. Work hard, work together, and stay R3BEL Fit! That is all.
Gym Rules

Members and guests must be 18 years old. No one under age 16 is allowed in the gym unless the owner (Alex) is present (insurance requirement). Parental guardian must be present with anyone age 16 or 17 who is training utilizing their parent’s membership.

Each member must scan their access card when entering the gym even if you are entering with a group (this allows us to provide the busiest hours in the future and it is also an insurance requirement).

Allowing unauthorized guests (no membership/waiver of liability) into the facility will get your membership revoked. Insurance doesn’t play so we can’t either.

The bay door on the upper body side can be opened during the summers on days when there is not a lot of pollen floating in the air. If you are the last person in the gym and the bay door is open, you are responsible to pull it closed and latch it before you leave.

No street shoes in the gym, you must have dedicated gym shoes.

Clothing is a requirement in the gym. Shirts on when lifting bro, can be off in the posing room.

No food in the gym. Eat outside.

Do not mix pre-workout or protein over the mats. It’s harder to clean up spills.

No smoking, vaping, chewing, or consumption of tobacco or marijuana products. This is a gym.

Emergency buttons are located near the entry of the gym if you need one while you are lifting. Put it back when you are finished with your workout.

Adhere to the manufacturer’s warnings and instructions on equipment.

Return all equipment to its proper place and wipe down machines after use. PUT WEIGHTS BACK WHERE YOU GOT THEM FROM

The dumbbells increase by 5 pound increments. There shouldn’t be 75 lbs next to 30lbs. Keep them in order.

Be respectful of other members lifting. Headphones in usually means people don’t want to talk. Ask first if you need to utilize the same equipment as someone else. Don’t just work-in without asking. If you need a spotter on a max lift just ask, most people are willing to spot real quick.

Don’t lean weights or bars against walls or equipment.

Use weight belts when lifting heavy.

Chalk is allowed but not provided as we do not want people pulling a LeBron James and clapping it up everywhere. You can use chalk but be respectful of others and clean up your mess (there is a vacuum in the entry if needed). Do not use chalk in the dumbbell area or on the cable machines.

Freelance personal training is prohibited (insurance requirement), however, if you are a certified and insured trainer and want to train clients, contact Alex.

The deadlift platform is specifically for deadlifting, as is the white barbell (Texas Deadlift Bar).

Load the 100 lb plates on the barbell at the same time. If you don’t, it will tip over and smash a foot.

No curling in the squat rack, we have curl bars for that.

Safety first! If someone’s form is whack, tell them! Don’t get offended, we are all trying to get better.

This is a judgment free zone (grunting is fine if that’s what it takes to move the weights).

Members will not touch the camera system and will not unplug any cords associated with it. You should only be messing around with cords that involve moving a fan, or moving the speaker, or a phone charger, etc.

To help keep costs down, turn off the lights and fans if you are the last one leaving the gym. Again, make sure the bay door is closed and don’t prop open the entryway door.

There are speakers in the gym and you are free to use your own, but be considerate of others. If you are lifting alone, blast the music as loud as you’d like. If others show up to lift turn it down enough so they can hear their headphones. If you are connected to the main speaker and are not lifting by yourself, remember that this is a gym and people don’t want to squat or deadlift to country music etc.
- If you need to cancel your membership, you must provide 3 days notice before your next billing cycle. Additionally, if you will be out of state for greater than 30 consecutive days, you can pause your membership dues while you are gone but you must contact Alex at least 3 days before your next billing cycle.

If another member is being obnoxious and not following the rules you have full permission to tell them to “knock it off,” “put on a shirt,” “put your shit back when you’re done using it” etc. Alternatively, you can contact Alex at 907-891-3447 and let me know and I will deal with it. By signing below, you agree that if you are acting like an asshole you are okay with another member telling you to act like an adult. Additionally, you understand that if you continue to act like an asshole I will cancel your membership. This keeps our gym functioning well and keeps everyone happy.
